Child Support Determination Form (Annex 1)

The Child Support Determination Form, or more commonly referred to as the Annex 1 form, is a form legally required to be completed when the Court is brought to establish child support based on the Quebec provincial child support guidelines.

In this form, you will need to indicate the sources of your income, the number of children that will need child support, some children expenses such as daycare and educational, and finally, you will indicate the custody arrangement that you have established with your ex-spouse.

This form will allow for the calculation of child support based on the payment frequency that you will have indicated.
